The NDIS has produced a survey for people who have reviewed or signed a service agreement or might do so in the future.
This survey is for:
participants
family members or carers.
Family members or carers includes people who may sign service agreements on a behalf of a participant such as guardians or plan nominees.
The NDIS wants to understand what:
helps people feel safe and confident when reviewing a service agreement
information and help people need before signing a service agreement
makes a service agreement clear, fair, and trustworthy.
Your feedback will help the NDIS know what would make service agreements easier to understand and more supportive for everyone.
This survey takes about 5 minutes. Your responses are anonymous, and you can skip any question you don’t want to answer.
A new way of planning
The NDIS has created the Design Hub to gather participant feedback on the new way of planning.
The Design Hub is where we test and refine:
new planning tools
resources
participant experience with the NDIA.
Design Hub sessions with participants started in October 2025. They will continue throughout 2026 as we prepare to launch the new way of planning from July 2026.
Using different methods to engage with participants, families, carers and the wider community, including workshops, surveys, and consultations with peak bodies and sector representatives.
